wedding vow renewal

What's the appropriate plan of a vow ceremony? Should I have bridesmaids and maid of honor,ect? Should I wear white if it's a regular evening gown? I've never been to one and I don't know what's supposed to happen. It's our fifth year anniversary. Please help!
